Civilian Personnel Working on U.S. / NATO Bases
Please note that according to the provisions of Circular note 1/2014, applications can be submitted up to six months before the start of the planned trip and, as a rule, no later than 15 calendar days before the start of the trip.

Visa applicants must make an appointment and must appear in person to submit their visa request.
You must submit ALL the required documents. The application form must be completely filled out and legible.
VISA FOR CIVILIAN PERSONNEL WORKING ON U.S./NATO MILITARY BASES IN ITALY
(Type of visa: mission)
This Consulate General will accept visa applications from legal residents of ARIZONA, NEW MEXICO, NEVADA and of the following counties in the state of CALIFORNIA: S. Luis Obispo, Kern, San Bernardino, Santa Barbara, Ventura, Los Angeles, Riverside, San Diego, Imperial Valley, Orange.
Civilian personnel (employees of the U.S. government) assigned to a U.S./NATO Military Base in Italy must contact the U.S. Department of State which in turn will submit the visa request to the Italian Embassy in Washington DC.
Civilian contractors/technicians working at a U.S./NATO Military Base in Italy must present the following documentation:
1) Original and a photocopy of your United States passport with a validity that exceeds the intended stay in Italy by at least three months. The passport must have two blank pages to affix the visa.
2) Original and a photocopy of your valid Driver’s License or State ID issued by California, Arizona, Nevada or New Mexico (Driver’s License or State ID issued by other States are not accepted); if you moved and have not updated your Driver’s License or State ID from California, Arizona, Nevada or New Mexico, you must submit a current (same, if available, or previous month of your appointment) bill of water, gas or electricity (other types of bill are not accepted). We may requested additional documents as proof of residence.
3) One visa application form, completely and clearly filled out.
4) Recent passport-size photograph (35 mm x 45 mm), on white background, full face and front view, in color. Scanned or photocopied photos will NOT be accepted.
5) Letters from the United States Sending States Office for Italy (USSSO) at the United States Embassy in Rome.
6) Letter from the US Department of Army, Department of Navy, etc. regarding your employment.
7) Copy of the contract between the U.S. government and the contracting company .
8) PAYMENT METHOD: Money Order or Cashier’s Check (payable to the Consulate General of Italy – Los Angeles) for the exact required amount. Please refer to the Handling fees for the exact amount.
9) Flight itinerary.
10) A self-addressed (from you to you) prepaid envelope (USPS, FedEx, Express Mail). Please fill out the authorization form. IF THE RETURN LABEL IS NOT COMPLETED PROPERLY (FOR EXAMPLE IF THE CONSULATE APPEARS AS THE SENDER) IT WILL NOT BE ACCEPTED.
11) a- For minor children, both parents (natural parents) must submit a notarized parental consent, addressed to the Italian Consulate, giving permission to accept the visa request and to issue the visa. If the parents are outside the United States their signature must be authenticated by an Italian Embassy or Italian Consulate in the country where they reside.
b- Certified copy of the Birth Certificates of the children. Foreign certificates must be translated in English and the translation must be authenticated by the Consulate (in the USA) of the country that issued the certificates. The original Birth Certificates must be attached to the authenticated translation.
This office may request additional documents that it deems necessary for the acceptance of the visa request.
The granting of the visa is contingent upon the authorization from the competent Italian Authorities. Therefore, it is advisable to submit a visa request not earlier than six months and not later than two weeks prior the departure date.
Within 8 days upon arrival in Italy, you must register with the local Police Department (Questura) to obtain the ‘Declaration of Stay’ (Permesso di Soggiorno).
The applicant must keep a copy of all documents that have been submitted with the visa application.
