Passports for Minors

In order to obtain a new Italian passport, minor applicants must be:

  • Italian Citizens;
  • Enrolled into our AIRE (Registry of Italian residing abroad) and reside in our consular jurisdiction of South California, New Mexico, Arizona and Nevada;
  • Have an appointment booked online through our PRENOT@MI booking system.


Passport applications for 
MINORS UNDER 12 YEARS OF AGE must be sent by  mail (see address at the top of the page) and do not require an appointment.

Applicants 12 years old and up must apply in person with the parents and therefore require an individual appointment.

DOCUMENTS FOR MINORS 12 YEARS OLD AND UP:

  1. Passport application form
  2. Pictures are normally taken in the Office at the time of the application, but we still recommend to bring one picture (US Passport size are acceptable) in case our new photolive system is down or malfunction.
  3. Previous Italian passport, unless you are a first-time applicant or if it has been lost/stolen, in which case you should file a Police report and bring a copy of it with you);
  4. USA Visa or Green Card, whichever applicable;
  5. USA passport, if applicable;
  6. Both Parents respective passports;
  7. Consent of the other parent only when not present at the time of the appointment. The consent cannot be older than 6 months. Please also provide a photocopy of the minor’s birth certificate in order to verify parentage

If the other parent is a citizen of Italy or of another Country in the EU the form does not need notarization but must be accompanied by a photocopy of his/her valid Italian or E.U. passport (page with passport holder’s information AND signature).

If instead the other parent is NOT a citizen of Italy of another Country in the EU, the signature on the form must be authenticated either by a public notary OR by an Honorary Consul of Italy in the Country of residency and accompanied by a copy of his/her photo ID.

If the other parent is deceased: provide a photocopy of pertaining death certificate.

  1. Passport Fee. Please consult our Consular fees: the amount corresponds to the line “RILASCIO ART. 27 (A + B)”. Payments can only be made with money orders or cashier’s checks made out to “Consulate General of Italy” and, in exceptional cases, with cash in the exact amount of the applicable fee. No personal checks or credit/debit cards are accepted;
  2. Normally, the passport will be issued the same day of the application. However, please bring also with you a pre-paid, self-addressed USPS Priority Return envelope (we do not accept Fedex, UPS or DHL) and fill out this mailing consent form. In fact in case of unforeseeable malfunctioning of the system or in very busy periods of the year the passport can be mailed instead or picked up in person when ready.

DOCUMENTS FOR MINORS  UNDER 12 YEARS OLD to be sent by mail only:

  1. Passport application form signed by both parents (if one parent is not an Italian or EU citizen, the signature must be legalized by a public notary and accompanied by the copy of his/her passport);
  2. Two passport pictures(Standard US Passport size are fine);
  3. Previous Italian passport, unless you are a first-time applicant or if it has been lost/stolen, in which case you should file a Police report and mail a copy of it with the application);
  4. USA Visa or Green Card, if applicable;
  5. Copy of US passport, if applicable;
  6. Copies of both parents’ passports;
  7. Passport Fee. Please consult our Consular fees the amount corresponds to the line “RILASCIO ART. 27 (A + B)”. Payments can only be made with money orders or cashier’s checks made out to “Consulate General of Italy”. No personal checks or credit/debit cards are accepted;
  8. Pre-paid, self-addressed USPS Priority Return envelope (we do not accept Fedex, UPS or DHL) and mailing consent form filled out and signed.

Please note: If one of the parent is deceased: provide a photocopy of the death certificate with the application.

The passport will be valid 3 years for kids younger than 3 years old; it will be valid 5 years for kids between 3 and 18 years old.
