The documents to be submitted are as follows:<\/p>\n
1) Form to Request the Registration in Italy of the birth certificate of a minor<\/strong> (click here<\/a>)<\/u>;<\/p>\n 2) Certified Copy of the CERTIFICATE OF BIRTH<\/strong> in \u201cLong Form\u201d format (it must contain all the exact personal information of the newborn child and the parents), issued by the competent local authorities (County Clerk Office<\/u><\/a>);<\/p>\n 3) Apostille<\/u><\/strong><\/a> (Declaration of international validity) to be requested only for the birth certificate issued by the \u201cSecretary of State\u201d of the State where the birth occurred (list of Secretaries of State<\/u><\/a>). The Apostille does not require translation<\/u>;<\/p>\n 4) Translation<\/strong> into Italian of the certificate, entirety, typewritten and faithful to the original in all its parts (list of Translators<\/u><\/a>). The format of the dates must be European type<\/strong>: day\/month\/yea<\/em>r;<\/p>\n 5) Photocopy of valid Italian passport of Italian parent and\/or any identification document in possession proving Italian citizenship;<\/p>\n 6) Photocopy of both parents’ passports.<\/p>\n <\/p>\n
