The declaration is free of charge and is issued by the Consulate upon request by the minor\u2019s parents residing in the jurisdiction of the Consulate General of Los Angeles and registered AIRE.<\/p>\n<p><strong>To request the declaration please email the below listed documents to <\/strong><a href=\"mailto:passaporti.losangeles@esteri.it\"><strong>passaporti.losangeles@esteri.it<\/strong><\/a><u>:<\/u><\/p>\n<ul>\n<li><a href=\"https:\/\/conslosangeles.esteri.it\/wp-content\/uploads\/2025\/09\/Modulo-richiesta-dichiarazione-di-accompagnamento_0001.pdf\">declaration form<\/a><\/li>\n<li>copy of the parent\u2019s passport<\/li>\n<li>copy of the minor\u2019s Italian passport<\/li>\n<li>copy of the accompanying adult\u2019s passport and permanent address<\/li>\n<li>airline and flight information<\/li>\n<\/ul>\n<p>Please note that if you are a non-European citizen parent, your signature must be notarized in front of a Notary Public in front of a Consular Attach\u00e9.<\/p>\n","protected":false},"excerpt":{"rendered":"Passport Department E-mail: Passaporti.losangeles@esteri.it Italian citizens younger than 14 years old must obtain a Declaration issued by the Consulate in order to travel alone or with people other than their parents.
