To obtain the authorization to ship human remains and\/or cremains into Italy, the Funeral Home must submit the following documents to the Italian Consulate. Please take into consideration that the approximate processing time is\u00a0ten (10) working days<\/strong>\u00a0from the day the all the proper documentation is received:<\/p>\n 1a.\u00a0Two<\/strong>\u00a0(2) certified copies of the\u00a0death certificate<\/strong> each one completed with \u201cApostille\u201d (the \u201cApostille\u201d is a legal document issued by the Office of the Secretary of State of the State where the death occurred, to be attached to the death certificate. Contact the Secretary of State for information concerning procedures and costs). 2a.\u00a0In the case of Ashes<\/strong>\u00a0a\u00a0certified cremation certificates in accordance with the procedures of the state where you operate (notarized, possibly on your own letterhead, they must bear your certificate of Authority Number and the cremation number) 3.\u00a0Only for shipment of full bodies<\/strong>: one (1) notarized Affidavit from the County Health Department stating that: 4. Information sheet (enclosure<\/strong><\/a>) completely filled out. Please write the name of the Cemetery in Italy, including the town and the province where it is located. The port of entry into Italy must be specified.<\/p>\n 5. Deceased Passport if in\u00a0course of validity or a certified copy of a foreign passport (only for Italian citizens the original is required if available).<\/p>\n 6. Letter from the family of the deceased (either residing here or in Italy) stating that the \u201cFuneral Home (complete name and address of the Funeral Home) has been chosen to conduct all the procedures related to the shipment of the body\/ashes into Italy, including all responsibilities involving the construction and the closure of the shipping box and the affixing of the Mortuary Passport and the Consular Tag (issued by the Consulate General of Italy) on the same box.<\/p>\n 7. Money Order in the amount of $\u00a0(please contact the Consulate to receive the updated\u00a0Consular fee. you may email the assistance office at\u00a0assistenza.losangeles@esteri.it<\/a>). Please note that consular fees are subject to change every 3 months. The fee is applied when issuing the final document, not when the request is submitted.<\/p>\n 8. Disposition\/Transit\u00a0permit issued by local Authorities<\/p>\n 9. Pre-adressed and prepaid return envelope to receive the death passport and consular tag (ONLY FEDEX or USPS).<\/p>\n
\n1b. In addition the\u00a0death certificate\u00a0MUST<\/strong>\u00a0be translated into Italian by\u00a0one of the translators you can find on our website.\u00a0(see\u00a0list of translators<\/em><\/strong><\/a>)
\nPLEASE NOTE<\/strong>: Both original death certificates with Apostille\u00a0WILL\u00a0NOT<\/strong>\u00a0be returned to the Funeral home or family. We suggest that you contact the airlines and confirm with them which documents need to be presented to them for the shipment of the remains\/cremains out of the US. This Consulate will provide the Death passport and Consular Tag.<\/p>\n
\n2b.\u00a0In case of full body repatriations\u00a0<\/strong>a certified statement that the Embalmers (include the license number) have embalmed and disinfected and embalmed the remains in accordance to local Board Standards (notarized, possibly on your own letterhead).<\/p>\n
\na) \u201cat the time of death of Mr\/Mrs \u2026.. there were no epidemic diseases within the County\u201d;
\nb) \u201cthe remains were embalmed in accordance to the laws of the State in which the Mortuary operates\u201d;
\nc) \u201cthe deceased did not die of any contagious disease\u201d.<\/p>\n
