Please note that it is not necessary to make an appointment for birth/marriage/divorce/death registration, simply send all the required documentation via FedEx or USPS to the attention of the Vital Records department at 1900 Avenue of the Stars, suite 1250; Los Angeles, CA 90067


Death Certificates of Italian nationals must be registered, through this consulate, to the Comune of last residency.

The documents needed to register a death are:

1) Certified Copy of Long Form of Certificate of Death translated into Italian;

2) "APOSTILLE" (Declaration of International Validity) issued by the "Secretary of State" (See Secretaries of State within our consulare jurisdiction here) of the state in which the death took place;

3) Application form to register in Italy death abroad duly filled out (available here);

4) Italian passport of the deceased.
